LIFE INSURANCE BENEFITS
Available to part time and full time hourly employees 6 months from date of
employment. Part time and full time salaried employees are eligible
3 months from date of employment.
PAID TIME OFF BENEFITS
Applies to all
hourly employees who are scheduled to work 20
hours a week or more 6 months from date of employment. Full time and part time salaried employees are eligible 3 months from date of employment.
PERSONAL DAYS
Applies to all
hourly part-time and full-time employees who are scheduled to work 20
hours a week or more. After ten years of employment, employees will receive 5 personal days per year.
These personal days must be used within the calendar year.
DEFINED BENEFIT
PENSION/PROFIT SHARING TRUSTS Applies to all employees who are at least 21 years old after one year
of continuous employment and who have actively worked at least 1000 hours
in that year. Defined Benefit Trust has been amended to include only those employees who have met the eligibility requirements by July 1, 2011.
Provides a means for employees who choose to contribute a defined dollar amount up to IRS contribution limits
on a pre-tax basis into a fund for retirement purposes.
MEDICATION ADMINISTRATION
INCENTIVE
A twenty-five cent an hour increase will be given to any hourly employee when they
become certified to pour meds. This increase will be forfeited if an
employee becomes decertified to pour medication.

EXERCISE REIMBURSEMENT* Provides a $100.00 reimbursement every 6(six) months to those employees
who actively participate in an approved exercise program. Reimbursement forms may be obtained on the CBS web site or from the Human Resource Department.

WEIGHT LOSS REIMBURSEMENT* Provides a $150.00 reimbursement to any employee that joins a bona fide weight loss program
(Weight Watchers, Jenny Craig, etc.). Evidence of membership and payment is necessary in order to apply. Reimbursement forms may be obtained
on the CBS web site or from the Human Resource Department.
* Total reimbursement for excercise and weight loss combined per year is a maximum of $150.00 per year.

PARTIAL COLLEGE
INCENTIVE REIMBURSEMENT Applies to all regular part-time and full-time employees who are
scheduled to work 20 hours a week or more. Reimburses up to $1000 for tuition per semester with a maximum
reimbursement of $2000 per calendar year.

DEFENSIVE DRIVING REIMBURSEMENT
Reimburses up to $60.00 for any employee who completes an approved New York
State Motor Vehicle Defensive Driving course.
DIRECT DEPOSIT
Payroll directs deposit may be done at employee’s request to his or her own
existing checking or savings account at any participating bank.
