MEDICAL, DENTAL AND LIFE INSURANCE BENEFITS
Available to full time hourly employees 6 months from date of
full-time/part time employment. Full time salaried employees are eligible
3 months from date of full/part time employment.
PAID TIME OFF BENEFITS
Applies to all
hourly part-time and full-time employees who are scheduled to work 20
hours a week or more after six months. Eligible employees will accrue paid
time off hours from the first day of employment. These paid hours cannot
be used for the first six months of employment;
thereafter; they can be used to make up hours not worked for sick, personal or
vacation. Accrued hours can be carried over as long as the employee uses
at least two weeks worth per year.
PERSONAL DAYS
Applies to all
hourly part-time and full-time employees who are scheduled to work 20
hours a week or more. After six years of employment, employees will receive one prorated personal day per year up to five days.
These personal days must be used within the calendar year.
DEFINED BENEFIT
PENSION/PROFIT SHARING TRUSTS
Applies to all employees who are at least 21 years old after one year
of continuous employment and who have actively worked at least 1000 hours
in that year.
The Agency will contribute to an eligible employee’s defined benefit trust
annually, and the funds will be managed by an investment company on behalf
of the Agency. Benefit statements will be provided to eligible employees
annually.
The Agency will contribute to an eligible employee’s profit sharing trust
annually, contingent on the Agency’s budget. These funds will be managed
by the individual employee through an investment company. Employees will
have access to view their statements on line and will be provided a
quarterly statement by the investment company.
If enrolled for 5 years, you will be considered vested and can receive
any benefits upon termination and/or retirement.
TAX SHELTERED ANNUITY
Applies to all employees
Provides a means for employees who choose to contribute up to 20% of their
income on a pre-taxed basis into an annuity fund for retirement purposes.
Please contact the Human Resource Department for enrollment package.
MEDICATION ADMINISTRATION
INCENTIVE
A 25 cents an hour increase will be given to any hourly employee when they
become certified to pour medication. This increase will be forfeited if an
employee becomes decertified to pour medication.
EMPLOYEE
ASSISTANCE PROGRAM
Community Based Services offers a free, confidential referral service,
with an outside provider, to all of our employees and their dependents
to assist employees with problems that can interfere with the ability to
function on the job effectively, efficiently and safely.
EMPLOYEE REFERRAL BONUS
Provides a $500 bonus for any employee referred to us by an
employee.
You must notify the Human Resource Department in writing when you refer an
applicant.
You will receive a bonus after a six-month period provided that you and
the employee that you referred are both actively employed at the time the
bonus is given out.
EXERCISE REIMBURSEMENT
As the incentive budget allows, employees are eligible for a reimbursement up to $100 every 6 (six) months for continued participation in an approved exercise facility. To receive the reimbursement, documentation of membership and participation of at least 50 times in the designated six month period is required (January – June, July – December). Reimbursement forms may be obtained on the CBS web site or from the Human Resource Department.*

PARTIAL COLLEGE
INCENTIVE REIMBURSEMENT
Applies to all regular part-time and full-time employees who are
scheduled to work 20 hours a week or more.
Reimburses up to $500 for tuition per semester with a maximum
reimbursement of $1000 per calendar year.
To receive this reimbursement you must:
- Be a full-time student (at
least 12 credits a semester or be matriculated in a course of study)
- Submit a signed contract and
proof of tuition payment to the Human Resource Department at point of
registration
- At the end of the semester,
submit a transcript of grades (you must have obtained at least a “C”)
for each course taken
- Three months after completion
of the course you will receive reimbursement provided you are actively
employed at the time of reimbursement
- Continuing education may be
reimbursed for employees who do not meet the above criteria at the
discretion of the CEO
DEFENSIVE DRIVING REIMBURSEMENT
Reimburses up to $60.00 for any employee who completes an approved New York
State Motor Vehicle Defensive Driving course.
DIRECT DEPOSIT
Payroll directs deposit may be done at employee’s request to his or her own
existing account at any participating bank.
WEIGHT
LOSS REIMBURSEMENT
Any staff that joins a bona fide weight loss program (Weight Watchers, Jennie Craig, etc.) will be reimbursed up to $150.00 annually for costs associated with membership. Evidence of membership and payment is necessary in order to apply. Reimbursement forms may be obtained on the CBS web site or from the Human Resource Department.
Total reimbursement for exercise and weight loss combined per year is a maximum of $250.00.
